Medicaid Waiver

The Medicaid Waiver Program is designed to assist people with developmental disabilities who cannot live independently. Services are divided into only one category:

Integrated Supports

Integrated supports refers to the battery of services provided to the individual consumer through the Medicaid Waiver Program. This usually means hiring an implementer, or one-on-one worker, who will provide direct care and assist the consumer to meet their annual goals and objectives. They may teach the consumer to budget his/her money, to shop, to do laundry, to cook, to do household maintenance tasks, or whatever the consumer requires. They often assist the consumer to participate in fun activities like basketball, bowling, community events, etc.
Focus will also assign an Integrated Supports Coordinator. It is the Coordinator's job to hire and supervise the implementer and to stay aware of and participate in all issues and decisions concerning the consumer.
At least once per year, the consumer's Waiver Team meets to discuss progress and needs for the future. This team may consist of the consumer, a legal guardian, family members, the case manager, the integrated supports coordinator, a representative from DDS, representatives from other agencies that serve the consumer and/or anyone that the consumer wishes to have present at the meeting. This meeting generates a plan for the next year that is sent in to the state for approval. If and when the plan is approved, it becomes the blueprint for services for the year to come.
The consumer is visited at least once monthly by both the case manager and the integrated supports coordinator to ensure that all needs are being met.

 

Medicaid Waiver Application

To apply for Medicaid Waiver services, contact your local DHS office. If you wish to choose Focus as your service provider, tell your DHS representative.
Note: Focus has a limited number of spaces in our "Focus Houses" which are alternative living units for people with developmental disabilities already on the Waiver Program. They are not group homes, but are more in the nature of apartments, in which adults live as roommates, with the necessary assistance from their implementers.
